MEMBER SECURITY CHECK

All Auxiliarists must undergo a degree of Security check. What you choose to become involved in determines the extent of the background check. There is no easy way to do this so I have tried to simplify the process by giving everyone a link to the forms. ALL BLANKS MUST BE FILLED OUT - you'll use a lot of N/A designations for not applicable.

You will need to complete the SF 85, SF 86, DHS for 11000, and OFI Form 86C plus be fingerprinted. Click on the link below to go to the forms.

New Members:

•  New Member Check-off Sheet (version Jan 2007)

•  Enrollment Application (01/07), ANSC-7001

•  Prospective Member Interview Record (11/97), ANSC-7036

•  New Member Course Guide (Draft)

•  New Member Reference Guide

•  Charting your Course for Service

•  New Member Exam

•  New Member Test Answer Sheet, ANSC-7010

•  Auxiliary ID Card Form

•  Personnel Security Information (PSI):

  •  Two (2) Fingerprint Cards: Form FD-258 (obtain from your Fingerprint Technician)

  •  Verification of US Citizenship Form

  •  All other security forms for new members are found on the above Enrollment Application (01/07)
